Home Care Aide

-Starting wage $17/hour for most DC cases, $15.20/hour for all other cases. All cases to pay $17/hour by February 2022.
-$500 new hire bonus (Paid in installments)
-Anyone working an average of 20 hrs/week eligible for benefits

Social Work Case Manager 

Type: Full Time or Part Time

Salary Range: $62,000-$67,000

Position Description: Home Care Partners has an opening for a Social Work Case Manager. The job involves home visits to provide support, counseling and information to family caregivers, case management for home care clients who receive long term home health aide services, and supervision for home health aides. Job Responsibilities:

· Client assessment and case management.

· Individual Counseling, information, referral and assistance for family caregivers

· Participation and facilitation of agency programs such as support groups and resource events.

· Documentation and tracking of home visits.

· Working with Home Health Aides to ensure that services to clients are satisfactory

Qualifications:

· Must have a car to make home visits in Washington, D.C. and be available during normal business hours (Reimbursed for mileage).

· MSW with a DC license REQUIRED. We will not consider candidates without a D.C. social work license.

· Strong documentation skills

· Experience with Seniors and/or Individuals with disabilities, family caregivers, and/or home health aides preferred

Experience with web-based databases a plus.

Nurse Case Manager 

Job Title: Nurse Case Manager

Type: Full Time or Part Time

Salary Range: Commensurate with Experience

Position Description: Home Care Partners has an opening for a Nurse Case Manager position. The job involves home visits to provide supervision and support to home health aides and case management for home care clients who receive long term home health aide services.  Opportunity to develop long term relationships in a flexible working environment. Job Responsibilities:

  • Supervision and evaluation of certified Home Health Aides (HHAs).
  • Client assessment and case management.
  • Documentation and tracking of home visits.
  • Communication with HHAs and clients.

Qualifications:

  • Must have a car to make home visits in Washington, D.C. and be available during normal business hours (Reimbursed for mileage).
  • D.C. RN license required.
  • Home Care experience or experience with Seniors and/or Individuals with disabilities strongly preferred.

Communications & Data Coordinator

Position Description: Home Care Partners has an opening for a Data and Communications Coordinator to work with a newly expanded program that supports family caregivers. Responsible for building solid relationships with clients, providers, and the community through regular communication via social media, newsletters, emails, phone blasts and other means. Responsible for regular database maintenance, monthly grant reports, and management of Caregiver Flex Accounts.

Salary Range: $50,000-$55,000

Job Responsibilities:

  • Provides outreach support, building relationships with key community stakeholders.
  • Provides data support, regularly updating tracking sheets and program databases
  • Communicates with clients, vendors, and other community stakeholders via multiple formats (i.e., email, text, social media, MailChimp, E-Newsletter etc.)
  • Prepares monthly grant reports and outcome measure documentation
  • Manages Caregiver Flex Account submissions and process reimbursements in a timely fashion.
  • Participates in the design, preparation and implementation of activities, events, and services for family caregivers.
  • Serves as initial point of contact for inquiries about the program.

· Provides courteous and prompt customer services to caregivers and community stakeholders

Qualifications:

  • Bachelor’s in a Human Services/Social Work, Communication, Public Health , Public Policy or related field.
  • Highly proficient in MS Excel and other MS Office programs.
  • Experience with MailChimp, ConstantContact, or other e-newsletter platforms preferred.
  • Experience with Microsoft Access or other databases a plus.
  • Experience with secure web-based data management systems preferred.

· Experience with Seniors and/or Individuals with disabilities and family caregivers preferred, but not required.
